Merge changes from topic 'demux-fragment-cleanup'

* changes:
  demux: remove GetFragment()
  demux: remove dead fragment related TODO
  demux, Frame: remove is_fragment_ field
  demux,WebPIterator: remove fragment_num/num_fragments
